Come and join our dedicated stroke rehab team working in patients own homes in West Hertfordshire.
Within the model we have the following:
Early Supported Discharge (ESD), high-intensity, home-based rehabilitation that a patient receives over up to 6 weeks following a discharge for mild-moderate new stroke.
Neurological Enhanced Transition Team (NETT), home-based high-intensive rehabilitation delivered up to 6 weeks following a discharge for moderate to severe new stroke or other neurological conditions.
Community Stroke Team, which is for patients experiencing a change in their daily function directly related to a pre-existing stroke injury.
This is a great opportunity for someone who is conscientious and adaptable. The successful candidate will have excellent communication skills and will be able to demonstrate an adaptable approach whilst meeting guidelines and stroke standards as well as being part of the new stroke pathway. They must be confident working as a lone worker, with the support of the team.
You will be working with people in their own homes, helping them to achieve their rehab goals, supporting therapy programmes set by Speech and Language Therapists, Physiotherapists, Occupational Therapist and Psychologists.
To be an active member of the CLCH ICSS multidisciplinary team, providing comprehensive therapy to residents within West Hertfordshire in their own homes. The ICSS includes Early Supported Discharge Team (ESD) Neuro Enhanced Transition Team (NETT) and Community Stroke team.
To work autonomously and undertake lone working in the community traveling to patient’s homes in and around West Hertfordshire
To minimise the effect of stroke on people’s lives by providing support to patients and their families during the transition home from hospital following a stroke, ensuring that they receive the right rehabilitation to optimise recovery.
To provide both clinical and administrative support to qualified members of staff, including occupational therapists, physiotherapists, speech and language therapists and clinical psychologists.
