Summary

Developing and implementing care plans for older adults with complex needs. Providing support and guidance to junior nurses and other healthcare professionals. Participating in ward rounds and multidisciplinary team meetings. Contributing to the development of policies and procedures related to older adult care. Leading or participating in quality improvement projects. Staying up-to-date on the latest research and best practices in geriatric care. Represent the Trust, and work collaboratively with CCGs and other stakeholders across the health community to share expertise and ensure patients are safely managed across the system. You will provide professional advice to the senior clinical staff in relation to older person inpatient care and act as an expert resource locally, regionally and nationally.
