Young Person’s Substance Misuse Worker
Location – Medway
Hours – 30 hours per week
Working Pattern – Monday to Friday (Flexible working pattern to be agreed with successful candidate)
Salary – £25,000 per annum pro rata

Job Summary & Purpose
The Young Person’s Substance Misuse Worker will deliver a substance misuse service to young people ensuring that individuals receive the appropriate interventions that meet their individual needs. You will be responsible for the assessment and management of a caseload as well as the delivery of both structured and semi structured interventions in both one to one and group sessions. You will co-ordinate effective transitions for clients, enabling continuity of care for existing young people who are making the transition into adult services.

What you need
– To have relevant professional qualification i.e. Youth and Community, Counselling, Social Work or NVQ4 Health and Social Care or equivalent.
– Have experience of working within substance misuse service with a clear understanding of the need for and ability to deliver quality services.
– To have experience in delivering assessments and developing and implementing care plans with young people.
– The ability to accurately update and maintain records, undertake risk assessments and deliver structured interventions.
– The ability to work with and manage challenging behaviour and maintain confidentiality at all times.
This position is subject to a Disclosure and Barring Service at Adults and Children Enhanced level
