The Post The Clinical Fellow would be expected to carry out duties expected of a core trainee doctor including assessing emergency ward attendees, clerking of patients admitted to the Maternity unit, labour ward and gynaecological wards, post-natal discharge checks for patients following forceps, caesarean section and other complicated maternity cases. The post holder will have the opportunity to assist in operating theatres, obstetric gynaecology and antenatal clinics. Labour Ward and Gynaecological Ward Duties When on duty the Clinical Fellow will be compulsorily resident and responsible for admitting all obstetric and gynaecological cases. First priority is to respond to all bleeps and calls from Labour Ward. When on labour ward duty, as first on call, it is not permissible to leave the Hospital or accommodation premises. Labour ward rounds take place at 08.30, 13.00, 17.00 and 20.30 with the Registrar. Any abnormalities on the antenatal ward should be notified to the duty Registrar. Gynaecology ward rounds take place every morning on Rayner ward with the team for each consultant Gynaecological Theatre and Outpatient Clinics The Resident Doctors are fully integrated into the work of the Obstetrics and Gynaecological Department. Admissions to Day Surgery Unit take place at 08.00. Admissions to Rayner Ward for main theatre lists may be on the day prior to surgery, or often at 08.00 on the morning of surgery. Gynaecology Clinics start promptly at 09.00 am in the morning and at 13.30 pm in the afternoon. Administrative Duties Time must be set aside for administrative duties including dictation of discharge letters for general practitioners. Prescribing The Hospital has a formulary a copy of which will be provided on commencement of duty. Teaching Friday afternoons are put aside for meetings and training. The hospital has a formal medical student training programme with Southampton University, with medical students being attached to the department. The hospital has an Audit office staffed by a team of clinical audit officers, who provide administrative and analytical support for audit projects. Research Doctors in training are encouraged to take part in research and audit projects. These are usually presented at the Friday afternoon postgraduate teaching sessions, or on audit days. The above does not purport to be an exhaustive list of duties but is intended to indicate the range and type of duties required. The duties of the post may alter to reflect changing practices within the service. The Rota The job is full-time with a 1 in 8 out of hours rota. As Jersey is an Island and unique in some respects, it is essential that the Department works as a team and all staff are required to be flexible and co-operative. If a Doctor is unable to attend work due to illness, they should inform the Department, the Consultant or in their absence the Middle Grade, and Medical Staffing immediately. If the absence continues beyond three days a medical certificate, signed by a doctor, is required. Gynaecology Service The gynaecology service carries out approximately 2,000 operations a year, and there are over 350 emergency cases a year admitted through the Gynaecology Ward. 38% of all operations are carried out in the award winning Day Surgery Unit that has recently been expanded. A Nurse-led pre-admission service operates for gynaecology. Each year there are over 6,000 referrals to the Outpatient Department, and there are over 1,000 inpatients admitted to the ward in a year. The Gynaecology Ward (Rayner Ward) has 16 inpatient beds, and attached to the ward is the Gynaecology Outpatient Department. Nursing staff rotate between the ward and the Outpatient area providing continuity of care. There is a fully equipped Colposcopy and Outpatient Hysteroscopy Unit. There is an Early Pregnancy Unit with an ultrasound scan machine. The Assisted Reproductive Unit (ARU) is located on Rozel Ward. It is led by a consultant, and assisted by a Senior Sister, a Staff Grade and receptionist. A satellite Assisted Reproductive service has been set up between Jersey and the Lister Hospital in London for egg recovery and embryo transfer for IVF patients. The Gynaecology Department carries out a considerable amount of laparoscopic surgery. Facilities for surgery are located in both Day Surgery and Main Theatre. The gynaecology outpatient department runs regular specialist clinics in Uro-Gynaecology, Colposcopy, Psychosexual and Gynae-Oncology. Jersey established a service to provide for termination of pregnancy after the relevant law was passed in 1997. Social terminations can be carried out until the end of the 12th week although in cases of severe foetal abnormality or serious maternal health problems terminations can be carried out until 24 weeks gestation. Depending on gestation, women are offered medical or surgical terminations. The termination service is led by a consultant and assisted by staff grade doctors, family planning nurses, counsellor and nurses with a special interest. There is an excellent community contraception service provided on the island with a dedicated family planning clinic at Le Bas Centre and for the under 21- year olds, Brook Jersey. There are close working relationships with the Genito-Urinary Medicine clinics. Psychosexual counselling is offered by one of the gynaecology consultants. Obstetric Service There are approximately 1,000 births per annum. There is a high quality, well equipped Labour Ward suite with intrapartum monitoring and blood gas analysis available. There is a designated Maternity Operating Theatre within the main operating theatre complex. There is twenty-four hour anaesthetic cover, with approximately 34% epidural rate. There is a complete mix of obstetric cases with the Unit having to deal with high-risk obstetrics as well as low risk because of the geographical isolation of the island. There are 4 Consultant Ante Natal Clinics per week. Most of the low risk antenatal care is shared between General Practitioner and the community Midwives. Each Consultant has an antenatal clinic each week for higher cases. Every antennal patient is offered a booking scan and combined screening for aneuploidy. All fetal abnormalities are recorded and the Unit is involved with the National Congenital Anomaly and Rare Registration Service (NCARDRS). Responsibility for fetal medicine is shared between two of the Consultants. Invasive investigative procedures including Chronic Villus Sampling and Amniocentesis are available on island. All obstetrics outcomes are monitoring and the department reports outcome data to MBRRACDE, and Each Baby Counts at the RCOG. All women are offered first trimester nuchal translucency scans and combined screening at the Ante-Natal Clinic. Antenatal and Intrapartum records are all computerised on a Trakcare system.
