Crynodeb
Provide expert clinical assessment, diagnosis, treatment, and evaluation of care for patients presenting with undifferentiated and undiagnosed conditions. Manage long-term conditions (e.g., asthma, diabetes, COPD) and implement individual care plans. Undertake same-day urgent appointments, triage, and manage minor illness clinics. Prescribe medications and treatments as an independent nurse prescriber. Review and action pathology results and hospital correspondence. Carry out audits, contribute to QI projects, and deliver key components of the PCN DES. Work collaboratively with GPs, pharmacists, HCAs, social prescribers, and the wider MDT. Contribute to supervision, mentoring, and education of junior clinical staff. Champion personalised care and population health management.
