Clinical Governance: To be responsible for the provision and development of clinical pharmacy services to Medical, Surgical and Health Care of the Elderly Wards in line with local and national Clinical Pharmacy Standards. To have overall responsibility for Medicines Information- co-ordinating and leading the departmental Medicines Information and Staff training programme in conjunction with the Pharmacy Manager, Principal Clinical Pharmacist and the Medicines Management Team. Investigate Clinical Incidents involving medications both within the Pharmacy dept. and throughout HSC, ensuring processes are in place to develop learning from these incidents. To lead on the participation in the NHS Benchmarking project, Medication Safety Thermometer and local KPI provision and monitoring. To be responsible for the review of Patient Group Directives within the Bailiwick of Guernsey and the implementation and updating of protocols, guidelines, policies, within HSC which are medicines related. Reviews results of external audit and benchmarking related to Medicines Information and develop action plan as necessary. To be a member of and contribute to the Clinical Guidelines Committee Aid in the provision of evaluated drug expenditure data To facilitate the undertaking and contribute to multi-disciplinary audit. Management : To deputise for the Lead Pharmacist Clinical Audit and Patient Safety in their absence in relation to day to day workload planning. To participate in meetings and working groups relating to new Drugs and Policies relating to Medication usage and to work closely with the HSC Clinical Governance Team, ensure that policies relating to medicines and clinical services are in line with local and national directives. To attend and advise HSC Governance Teams of any Safety Issues surrounding the provision and administration of Medicines within HSC and to report to Governance internal KPIs and external Benchmarking Information. To provide advice to medical staff on medicines usage and expenditure. To be a member of the HSC Therapeutics Advisory Committee and the Prescribing and Formulary committee Leadership: To act as the Pharmacy Lead for the provision of Medicines Information and Training To lead in the Clinical Pharmacy Support to the Electronic Prescribing Project in the area of Orthopaedics and Care of the Elderly To provide professional leadership to the Medicines Management team. To be the secretary of the Therapeutics Advisory Panel and cover for the Chair in their absence Service Development: Develop the Medicines Information Service for the Pharmacy service area and participate in the development of a Staff Training Programme. To be responsible for the development of a strategic plan for Medicines Information Develop and monitor services to support an improved discharge service for patients, and carers ensuring a more effective transition from Secondary to Primary Care. Initiate, plan, develop and implement clinical guidelines related to medicines use within HSC. Education and Development: Plan and deliver training to pharmacy and other clinical staff on issues related to New Medicines or any issues raised as a result of Clinical Audit. Supervising students on wards as appropriate. To provide training to the Medicines Management Team in the form of Clinical CPD Meetings. To provide training to ward and HSC staff, including the annual prescribing update to consultants employed at HSC. The post holder will be committed to the concepts of continuing professional development and lifelong learning personally and for staff. Contributing to the induction and competency based training of Clinical pharmacy, Medicines and medical and nursing staff.
