Job title: Senior Experienced Clinical pharmacist Mode: On site during probatory period then transition to hybrid Salary: Depends on experience (band 8a) Job Summary: We are seeking a highly skilled and experienced Senior Clinical Pharmacist to join our Primary Care Network 7, which consists of Oak Lodge Medical Centre and Mulberry Practice. The successful candidate will be predominantly based at Mulberry Practice and report directly to the PCN Lead Clinical Pharmacist and the Lead PCN clinical directors. You will work collaboratively with a dynamic team of pharmacists and a pharmacy technician to deliver exceptional patient care and optimize medication management. You will assist with the following key areas: Chronic conditions management and clinic provision in areas including hypertension, diabetes, asthma, COPD etc Experience in lipid management and ability to initiate/monitor cholesterol lowering medications during cardiovascular medication reviews Able to support the practice to achieve its QOF targets / and other practice targets which involve prescribing using clinical protocols Support pharmacy technician and junior pharmacist with clinical and pharmaceutical problems and provide guidance on complex medicines-related queries Provide expertise in clinical medicines review and address public health and social needs of patients in GP practices Collaborative working as part of PCN7 prescribing hub Reduce inappropriate polypharmacy and wasteful prescribing through clinical medication review Improve the incidence of medicine related hospital admissions and readmissions by supporting patients to get the best outcomes from their medicines Liaise with community pharmacy and hospital pharmacy teams to ensure continuity of care and resolution of medication queries Manage the repeat prescribing re authorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates Manage patients and healthcare professionals medicine queries Increase quality and safety of prescribing through mechanisms such as audits and recall systems Manage practice formularies to improve the quality, safety and cost effectiveness of prescribing Implement drug withdrawals and alerts e.g. MHRA aimed at improving medicines safety Work with primary care professional and patients to implement NICE and other evidence-based guidelines Run clinics where medicines are the main intervention including DOAC monitoring Provide medicines information and training to Practice healthcare professionals and admin staff Act as a source of medicines information for all of the practice team and patients (e.g. around doses, side effects, adverse events, possible alternatives e.g. around out of stocks) Benefits: Competitive salary and benefits package. Opportunities for professional development and career progression Free onsite parking Primary roles and responsibilities Medication Reviews Hold clinics for patients requiring face-to-face structured medication reviews i.e. a review of the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicine taking. Provide a telephone support for patients with questions, queries, and concerns about their medicines. Undertake structured medication reviews with patients with multi-morbidity and polypharmacy and implement own prescribing changes (as an independent prescriber) and order relevant monitoring test. Long Term Conditions See patients in long term condition clinics and implement improvements to patient's medicines, including de-prescribing. Manage own case load, run own long-term condition clinics in conjunction with GPs and support clinical decision making when medicine management input is required Review the ongoing need for each medicine, a review of monitoring needs and an opportunity to support patients with their medicines taking. High-risk Medication Reviews Manage patients at risk of hospital admissions and harm from poor use of medicines. Undertake clinical medication reviews with patients with multi-morbidity and polypharmacy and implement own prescribing changes (as an independent prescriber) and order relevant monitoring tests. Attend and refer patients to multidisciplinary case meetings. Repeat Prescribing Produce and implement a practice repeat prescribing policy. Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates. Ensure patients are booked in for necessary monitoring tests where required. Management of Medicines at Discharge from Hospital Perform a clinical medication review, produce a post discharge medicines care plan including dose titration and booking of follow up tests and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge. Collaborative working Collaborative with the existing PCN pharmacy team to deliver on PCN targets, including CQC, Payment Quality Scheme (PQS) and other relevant workstreams Provide regular feedback to improve the working relations of the pharmacy team across both sites. Cost Saving Make recommendations for, and manage pharmacy technicians to, make changes to medicines (switches) designed to save on medicine costs where a medicine or product with lower acquisition cost is now available. Clinical Advice and Expertise on Medicines Answers all medicine-related enquiries from GPs and other practice staff with queries about medicines. Suggesting and recommending solutions. Provide telephone advice for patients with questions, queries and concerns about their medicines. Providing follow up for patients to monitor the effect of any changes. Care Quality Commission To ensure the practice(s) are compliant with CQC standards where medicines are involved. Training Provide education and training to the Practice staff on therapeutics and medicines optimisation.
