The CNS is the Keyworker, providing contact and information. The CNS will be present in Outpatients, will visit patients on the wards when having surgery, and liaises closely with other professionals to ensure seamless co-ordination of treatment pathways when patients are having chemotherapy and/or radiotherapy in other Trusts . They are a core member of the MDT (multidisciplinary team), and present and record outcomes. They undertake Holistic Needs Assessments (HNA's). They provide teaching to other members of the nursing staff.
