We are looking for enthusiastic and motivated individuals to join our expanding Urgent Response / Hospital at Home and HOPT Team based in a community setting. This is an exciting opportunity to be involved in the Integrated Health and Social Care Organisation.
You should have excellent communication and interpersonal skills which will be utilised with in this multi-disciplinary health care team. You must be multi-skilled and be able to work across various disciplines.
You must be proficient in providing planned care and be able to take appropriate action in achieving set goals with minimum supervision.
It is essential that you are able to fulfil the travel requirements of the role and vital that you hold a full UK driving license and have access to a vehicle.
The Nurse Associate will provide supervision and assessment for Healthcare Support Workers and below undergoing relevant training.
You will be given a dedicated induction programme and a full training and development plan will be offered.
You will have a formal Nurse Associate qualification and be practicing as a Band 4.
We welcome forward thinking individuals who would like to be part of a dynamic crisis team covering the city of Salford to support a discharge to assess model.
Due to the nature of the post it is expected that you will have a full driving license and will have regular use of an appropriate vehicle (as determined by the Trust).
