Job Title: Community Engagement Worker – Annual Health Checks
Hours & Salary: 30 Hours Per Week - £29,400 (Pro-rata)
Employment Status: 3 Years (Fixed Term)
Responsible To: Project Lead (Annual Health Checks)

Location: West Midlands Region (Community Based)

We're seeking a compassionate and proactive Community Engagement Worker who is passionate about improving healthcare access for people with learning disabilities. You'll be the perfect fit if you:

• Can adapt information creatively to make health checks understandable and accessible.
• Truly believe in inclusive, person-centred approaches that empower individuals.
It would be a bonus if you have:
• Experience in primary care, social prescribing or community health
• Knowledge of the Annual Health Check system
• Lived experience of disability or caring for someone with additional needs

What does this role involve?
This role is a vital part of a targeted health inequalities project aimed at improving access to preventative health care for people with learning disabilities. You’ll work directly with individuals and families while collaborating with Primary Care, community organisations, and public health partners.
A key element of this project is the guidance and oversight of a Steering Group made up of:
• Primary Care and statutory partners
• Voluntary, community, and faith-based organisations
• And most importantly, people with lived experience of learning disability or caring responsibilities
The Steering Group will support you and the wider project to ensure all activity is shaped by real voices, reflects local priorities, and remains accountable to the communities it serves.
Key responsibilities.
Community Engagement & Outreach
• Work directly with individuals with a learning disability, their families, and carers to promote awareness and understanding of Annual Health Checks.
• Deliver inclusive sessions in community venues, day services, support hubs, and public spaces.
• Provide one-to-one support and peer-informed outreach to build trust and increase confidence in using health services.
Promotion, Accessibility & Awareness Campaigns
• Co-design accessible promotional materials (including Easy Read, visuals, and digital content).
• Promote AHCs through outreach events, local media, social media, and community champions.
• Ensure messaging is tailored to meet the cultural and communication needs of diverse communities.
Partnership Development
• Build and maintain strong working relationships with:
o Primary Care stakeholders, including GPs, practice staff, PCNs, and Learning Disability Health Facilitators.
o Statutory agencies such as ICBs, NHS community teams, and local authorities.
o Voluntary, community and faith-based organisations who can support wider engagement.
• Facilitate aligned working and shared campaigns to ensure consistent and inclusive messaging.
Steering Group Collaboration
• Work closely with the project Steering Group, participating in regular meetings and using their insights to shape project delivery.
• Ensure people with lived experience have genuine input into the project’s planning, materials, and evaluation.
• Act as a conduit for feedback between the Steering Group and wider community voices.
Monitoring, Learning & Impact
• Record engagement activity, participant data, and feedback using agreed tools.
• Collect real-life stories, examples, and case studies to demonstrate impact.
• Contribute to written reports, dashboards, and presentations for funders, partners, and internal use.
Support evaluation and reflective learning to inform future delivery.
