Develop and maintain an understanding of the Cancer Waiting Times guidance to ensure that two week wait, 28 day, 31 day and 62 day targets are met To monitor and accurately track patient on a suspected cancer pathways via the Somerset system. Ensure actions are chased and reviewed prior to the weekly cancer PTL meeting To monitor and track upgraded patients via Somerset To be responsible for checking the outcomes of patients attending clinic on a suspected cancer pathway and monitor bookings of required tests within deemedtimescales To work with the team to resolve delays in the pathway at the earliest opportunity and where this is not possible escalating issues in accordance with Trust and networkprocedures To assist in meeting timescales of the submission of data for national and localrequirements of the Cancer Network. This includes, but is not limited to, data relatingto CWT, COSD and the national cancer audits. To contribute to the Peer Review (QSIS) process by providing required audit data(e.g. MDT attendance data) To assist in the validation of all mandatory CWT fields for treatment records forsubmission to NHS Digital within the given reporting month/quarter. Attend the weekly PTL review meeting with the Service Manager for Cancer againstthe PTL SOP requirements. Escalation of issues to enable relevant actions to be taken to overcome problems with meeting targets To assist with composing draft breach reports for all CWT treatment breaches. Theseshould include: root cause analysis; chronology of events; recommended actions;evidence of implementation of recommendations To assist in the running of weekly MDT follow up meetings with the MDT LeadClinician and maintaining attendance records To assist in the accurate input of the outcomes of the MDT meetings into the Somerset system Assist in the preparation and circulation of lists of patients for discussion at eachmulti-disciplinary team meeting (MDT) using information provided by relevant parties. To assist with the preparation and collection of all documentation, information,medical records, imaging, and pathology required to support each meeting, thus enabling the smooth running of the MDT meeting Where necessary to liaise with other Clinicians and MDT Coordinators/administrators outside of the Trust in relation to Tumour sites where the Trust is a Tertiary Centre To ensure the timely and efficient transfer of patients and information between trustsinvolved in the patient pathway, adhering to agreed communication pathways andprotocols To assist in ensuring that all operational changes affecting the MDT are communicated effectively with the whole MDT and other interested parties To assist in checking with the MDT Lead Clinician that all the information is correct and complete. To acquire Lead Clinician sign-off of all MDT outcomes Assist in setting up tumour specific cancer service meetings Use Patient Administration System to book patient outpatient appointments Cross cover colleagues within team to ensure tracking, MDT meetings are covered Work closely with the Team, Tumour Cancer Lead, Consultants & CNSs. Internal teams from other departments such as Admissions, Clinical records. Liaise with patients and carers Establish good communication channels with external teams such as GP Surgeries/Other Trusts.
