As a member of the Stroke team, the post holder will take a key role in leading and developing the Stroke SSNAP requirements across the Stroke Service at Epsom and St Helier Hospital.
The post holder will contribute to the provision of data quality for stroke patients, within the multidisciplinary team, ensuring that high quality administration is delivered within the Trust’s Stroke services. In conjunction with the multidisciplinary team, the post holder will record and monitor stroke patientoutcomes.
The post holder will provide advice and support to patients, their carers and other health care professionals. The post holder will work within agreed care settings according to established protocols or guidelines, and will contribute to the smooth running of the Service from an inpatient and outpatient setting.
The post-holder will be expected to take a lead role in the collection andanalysis of mandatory national Stroke data and the dissemination within the Stroke teams and Medicine Directorate. They will be involved in facilitating Stroke Service Improvement to streamline the patient journey and improve the quality of patient care, and also in developing information systems, in line with Trust strategy.
To support the clinical team with timely and accurate data collection and management of the Stroke data for internal and external submission.
MaintaineffectiveworkingrelationshipswiththewiderStroketeam, enabling joint working initiatives between both sites.

1. DutiesofthePost
1.1 TobeafullyactiveandcommittedmemberoftheStrokemultidisciplinaryteam.
1.2 Toleadandcoordinatetimelyandeffectivepathwayplanningforstroke patients from the point of admission,
1.3 Toleadandsupporteffectiveandtimelycommunicationbetweenthe Trust, Multidisciplinary Team (MDT) and external agencies involved throughoutthepatientpathway,inordertoensureahighstandardof stroke specialist care and to support the Stroke MDT.
1.4 Tosupportthestrokenursestoactivelyinvolvestrokeserviceusersin feedback of their experiences on the stroke pathway, utilising this information to develop both new and existing ways of working.
1.5 To act asa resourceperson toprovide accurate information, advice and support to thehospitalstaff,patients, carersandoutsideagenciesabout issues relating to the patient pathway
1.6 Ensure patient records are completed accurately and contemporaneously;usingTrustITsystemswhenrequired.
1.7 HaveanawarenessofHealthandSafetywithintheworkenvironment.
1.8 To be aware of the Data Protection Act and maintain patient confidentialityaswellasmaintainingthepatient’sprivacyanddignity.
