Crynodeb
Provide basic pharmaceutical advice to the primary health care teams/patients/carers in accordance with NICE and other national guidance and local guidelines. Communicate medicines optimisation issues for individual patients to the primary health care team. Attend practice meetings at allocated practices and contribute to the discussion and formulation of a practice prescribing action plan for the coming year. Undertake practice based projects / audits to support work within the practice prescribing action plans and locality plans. Undertake other practice based projects to improve prescribing performance ensuring compliance with the Bolton locality prescribing strategy. Support practices to achieve their quality and cost improvement targets as specified in the locality prescribing improvement program. Work with practice staff to develop practice-specific repeat prescribing policies.Audit repeat prescribing systems and advise practices on how systems could be improved. Work together with the practices during the implementation of the new external medicines management support systems to help ensure the accurate transfer of medicines information (examples included but are not limited to: Business Intelligence Portal, ScriptSwitch).
