Our multidisciplinary teams work flexibly to provide a 7-day service, 365 days a year, between 8am and 8pm. Patients who are discharged from hospital into Home First will be receiving step-down intermediate care, involving community-based assessments and interventions provided in their own home to support recovery and monitoring of their health in the early days post discharge to prevent re-admission.


You could become part of our exciting service, bringing the unique benefits of a wraparound recovery service from hospital discharge. The principle of the service ensuring that recovery involves working towards the person being as independent as possible through enablement.

The focus of this role is providing prompt and efficient co-ordination and administration support to a multi-disciplinary team (MDT).





You will be the point of contact for service users,theirfamilies,and carers when information on theircrisis andshort-term care support isrequired. This will include enquiriesaboutanticipatedvisits,providing updates, and listening to theirconcerns.





A key element of your duties will be processing and administering referrals using criteria guidelines and seeking advice from relevant team members when required. You will sign-postthese referralsto MDT members and other services asappropriate whilemonitoring, tracking, and reporting on progress within the patient pathway. This will involve escalating any issues or delays to the Operational Team Lead forappropriate action.
