Based in the Sherbourne team, you’ll also work closely with local Primary Care and Access & Wellbeing services, ensuring holistic, joined-up care that makes a real difference.
Working within a compassionate multidisciplinary team, yourNurse Specialist Practitionerexpertise will be central to shaping care plans, leading on safe and effective prescribing, and supporting evidence-based interventions.

This permanent, part-time role is for 15 hours per week, with flexibility on how those hours are worked, to be discussed at interview.

The role of theNurse Specialist Practitionerwill be to provide specialist clinical interventions to service users. It also involves delivery of leadership activities within the team.
Undertaking the assessment, planning and delivery of direct patient care at an advanced level with minimal supervision, ensuring continuity of practice and working within an MDT and multi-agency environment. Medication review and prescribing, and the provision of up-to-date and accurate prescribing advice. Provide clinical leadership and oversight of referrals, assessment and caseload management. Management and mentorship of clinical staff as required in line with the NMC Code of Conduct and the Trust's line management policies and procedures. Ensuring that systems are in place to co-ordinate information about the take up of the services/treatment pathways and to establish unmet needs and action plans to address those. Undertake other duties delegated by the Operational Manager in keeping with the scope and authority of the job description. Ensure through effective leadership and management, continuous service development, improvement, and high-quality service delivery. To work closely and in partnership with colleagues within our CMHT, Primary Care and their community partner and VSCE organisations in a collaborative way, ensuring that decisions are made that ensure the best care for service users and that there is a seamless pathway.
