Interview method: In person This role is subject to: Casual Car Documentation - The successful candidate will be a designated Casual Car User which means that car journeys may be necessary on an occasional basis, but are not essential to the role. If you do drive, you must always hold a full valid driving licence and provide your own car for use at work, as well as provide all the appropriate documentation. You will receive an allowance per mile of business use. DBS (Disclosure and Barring Service) clearance - this post is covered by the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (amended in 2013). Appointable candidates will be required to apply for an Enhanced Disclosure and Barring Service (DBS) check. Information provided by you or the DBS will be dealt with in a confidential manner and in accordance with the DBS Code of Practice which can be viewed here. We have a policy on the Employment of Ex-Offenders and it is available upon request. Evidence of conduct in previous employment concerned with the provision of services relating to. Health or social care or Children or vulnerable adults Proof of identity including a recent photograph Full Employment history

The Complex Needs and Preparing for Adulthood Service is made up of several multi-disciplinary award-winning citywide teams which sit within Manchester Adult Social Care, supporting vulnerable and multiply excluded citizens develop fulfilled, meaningful and safer lives.

The Service is made up of several, specialist teams which include, Preparing for Adulthood, The Drug and Alcohol Team, Entrenched Rough Sleepers Team, The Sensory Team, The No Recourse to Public Funds Team, The Direct Payments team and The Emergency Duty Service. We are also developing the Autism and Personalisation team.

Over the last 3 years, significant investment has been provided to the Complex Needs Service, and within each team we have several specialist roles which include, an Autism Lead, Social Workers, Approved Mental Health Professional, Social Care assessors, Person centred planners, Link Workers, Vision Rehabilitation Specialist, Specialist Sensory equipment assessment and installation officers, Direct Payment Coordinators, Assertive Outreach Workers, No Recourse to Public Funds Coordinators, Service Improvement and Development Leads and Project Officer.

Each role profile has a specialist training plan, to ensure staff are skilled and offered continued development. There are also lots of opportunities for career progression through our apprenticeship progression schemes.

The Complex Need and Preparing for Adult Service is an exciting landscape to work in, which is evidence based and working on the cutting edge of social work. If you like fast paced, innovation work where you will be given autonomy but high levels of support, this is the Service for you. This team doesn’t say we can’t…. it says how can we.

The Manchester Complex Needs and Preparing for Adult team, have developed an exciting team of Person-Centred Planners who will work alongside the Social Work Team and directly with individuals. These are diverse roles, which need highly skills people to ensure we provide gold standard support to the people we work with.

The Person-Centred Planners will work with a diverse groups of people, including young people, people with Autism, Learning disabilities, physical disabilities, Autism and Neurodivergence, Young People who are hard to reach and or at risk of exploitation and adults with drug and alcohol addictions.

Within the role you will be trained to a high standard and you will take part in a number of roles, which include:

  • Person centred planning (supporting people to be in control of their lives, have their voice heard, and plan how they want there needs met)
  • Coproducing with Citizens Strength Based, Person centred Care Act assessment
  • Providing assertive outreach to citizen – empowering people to access support and reach their potential
  • Deliver and develop projects, for example young people groups, developing accessible information, community profiling.

You will work across the city and develop links with with key stakeholders in children’s and education services, health and the voluntary, community and social enterprise sector to support

This role is an exciting opportunity to work across these services to play an integral role in supporting our warm welcome approach to adult social care services, providing accurate and timely information, advice, and guidance to people in Manchester. There will be scope for the role to focus on different project areas, including but not limited to:

  • Supporting how we work with care leavers
  • Actively promoting the use of direct payments in innovative ways
  • Using the local offer to support young people with additional needs
  • Developing our approach to transitional safeguarding
  • Developing our offer to Autistic and Neurodivergent Citizens

The successful candidate will be part of the Personalisation Team, working across Complex Needs and Preparing for Adulthood Service.

You will need to be well organised with proven experience of planning and managing competing priorities. You will have good communication skills as a key element of the role is the ability to build relationships and work with Manchester citizens and partners across health and social care to interpret and deliver defined plans and activities.
