The Rapid Access Care Unit (RACU) is a Community Same Day Emergency Care (SDEC) unit for medically unwell patients. We are based at Townlands Hospital in Henley-On-Thames.  RACU offers a full Multi-Disciplinary Team (MDT) assessment and treatment for patients, aiming to reduce hospital admissions and to care for patients as close to home as possible. We are part of the Integrated Community Care (ICC Pathway) and work closely with many community services for the benefit of our patients.

We are looking for an enthusiastic Band 6 Team Leader to help lead our wonderful service. 

Main duties of the job

Successful applicants will undertake a variety of roles including being in charge of the unit, working directly with patients in the SDEC and conducting home visits and assessments, delivering a high standard of care to all patients. They will need to be flexible and adaptable in order manage the complexities and unpredictability of the SDEC.

The Team Leader will provide strong leadership and direction for the team, monitoring and maintaining standards and professional practice. Act as a professional role model and provide support for other staff and students. To be a visible clinical leader, promoting team work and high standards of care within the experienced MDT in order to achieve the best outcomes for our patients.

Detailed job description and main responsibilities

As a band 6 Team Leader you will:

·        Coordinate and be in charge of the daily activity of the unit. Take and manage referrals, delegate work load to staff, ensure care is completed and documented. 

·        Supervise junior staff and students. Be responsible for staff supervision, PDR reviews, managing sickness etc

·        Directly care for patients. Conduct a thorough patient assessment and discuss with medical team regarding care planning and treatment. Give patient treatment and appropriately monitor and evaluate treatment progress. Recognise and respond appropriately to deteriorating patients. 

·        Partake in audits and service improvement to improve clinical practice and ensure standards are met.
