Crynodeb
Supports health promotion and prevention activities across the PCN, including smoking cessation, weight management, and lifestyle interventions Plays a key role in vaccination and immunisation programmes within the PCN Works within a Multi Disciplinary Team to support patients with long-term conditions and health inequalities; Provides clinical support to registered nurses and other healthcare professionals within the PCN; Delivers basic clinical care interventions under supervision, supporting continuity of care and patient safety initiatives. Taking and recording Blood Pressure, height & weight Performing ECGs and Phlebotomy Assisting with chronic disease monitoring Conducting health checks Assisting GPs and nursed with minor procedures Updating patient records and ensuring accurate documentation Personal/Professional Development: Post holder must hold a care certificate or work towards one Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work Inform the lead GP of any concerns regarding role and any professional development needed Be aware of own professional boundaries and what to do when you have reached them Quality: The post-holder will strive to maintain quality within the Practice, and will: Alert other team members to issues of quality and risk Work to practice protocols Assess own performance and take accountability for own actions, either directly or under supervision Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the teams performance Work effectively with individuals in other agencies to meet patients needs Effectively manage own time, workload and resources Contribution to the Implementation of Services: The post-holder will: Apply Practice policies, standards and guidance Discuss with other members of the team how the policies, standards and guidelines will affect own work Participate in audit where appropriate
