Are you an experienced nurse or therapist, passionate about improving the care of people with Multiple Sclerosis (MS) ? We are looking for an enthusiastic individual to join the Adult Community Rehabilitation Team (ACRT) and become the MS Specialist practitioner for City and Hackney. You will have experience working with people with MS, ideally having worked in acute and community settings. This is a full time permanent post.

By adopting a holistic, collaborative and coordinated approach to improve the experience, quality of life and survival of patients with Multiple Sclerosis in City and Hackney, Through the application of specialist clinical skills and evidence-based practice the nurse/therapist will ensure appropriate clinical interventions at each stage of the MS disease process to optimise quality of life.

Provide specialist advice and support for symptom management to optimise quality of life and engage in self management techniques.

Effective networking and appropriate referrals across health, social services and the voluntary sector the post holder will optimise the available resources to achieve a seamless care pathway reducing the potential for non-elective hospital admissions and institutional care.
