This new role within our pre existing Pulmonary Rehabilitation (PR) team, plans to provide greater ease to the patient pathway through PR.

The Pulmonary Rehabilitation service delivers individual exercise in a group / class setting to patients referred to the service with a respiratory condition. Providing a triage, individual pre assessment and post PR assessment to ensure appropriate referral and to gain patient outcomes.

Integral to the service is entering data for a national audit programme. pulmonary rehab runs on an 18 week waiting pathway.

The role of the assistant practitioner is patient facing and delivering post PR assessments once competencies have been completed, to assist and deliver group based exercise and educational programme twice a week over an 6 week period at a variety of sites across the mid Nottinghamshire.

The role is primarily concerned with delivering group based exercise and educations sessions to adults diagnosed with a respiratory condition across in Mid Notts. This includes maintaining accurate records and supporting the wider team to safely manage the waiting lists.

This role requires excellent communication skills both digital and verbal and a confidant manner when speaking to the public.

You will be required to have some knowledge and experience of working with people diagnosed with dementia or other mental illness and be able to offer support and care to them whilst they are attending the group.

You need to be able to work effectively within a team whilst being supported by registered staff.

The assistant practitioner will work alongside colleagues to prepare and deliver group based exercise and education interventions in a variety of settings, across Mid Nottinghamshire to patients refereed into the service with a respiratory condition diagnosis

You will be confidant speaking to members of the public and be able to modify your communication style as required to the situation.

You will have an understanding of respiratory conditions and rehabilitation, ideally with experience of community working and be able to provide care and support as required.

You will act professionally at all times and demonstrate a positive image of the service.

You will maintain accurate records and know your limitations and when to escalate risk and safeguarding concerns to the registered team members.

You will support the team to patients and carers on the waiting list for the service.

You will be involved in all aspects of the service and team working as required.

During your induction, you will be supported by registered staff and full training will be given including monthly clinical supervision.

Please note that this post does not meet the pay level required for a Skilled worker visa. Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.
