Crynodeb
This role's purpose is to support the commissioning of safe and effective services for people with learning disabilities. This involves supporting discharge from the hospital and the oversight of commissioned community packages of care, thereby preventing hospital admission. It will involve participating in the assessment, planning, and implementation of proactive care and case management for Brent's population with a Learning Disability or autism. The post holder will be the main point of contact for health and social care colleagues, providers, service users, family, and carers with the appropriate placements and facilitate the care system to move people through the system regarding the Transforming Care Concordat. The post holder will be a Brent Community Health Team member for People with Learning Disabilities within the Learning Disabilities Service of Central and North West London NHS Foundation Trust. They will report to the Service Manager and be designated for coordinating and delivering the Transforming Care Programme. The potential candidate will need to possess good communication skills, both written and verbal, and feel able to easily communicate with a range of stakeholders. Confidence in managing and maintaining credible and flexible working relationships with partnership providers and stakeholders is essential, as is the ability to demonstrate experience and understanding of the Learning Disability Service.
