You’ll be a core member of our Crisis Care team, delivering rapid, community-based support through Rapid Response, Home Intensive Treatment, and medical discharge pathways—helping prevent hospital admissions, support safe returns home, and empower young people to stay in their own homes.

Key Responsibilities:
• Assessment & Intervention:Conduct psycho-social and risk assessments, create tailored care plans, and deliver creative interventions to support mental well-being.
• Multidisciplinary Collaboration:Work closely with CAMHS, acute and specialist hospitals, police, education, and social care to provide coordinated support.
• Crisis Management & De-escalation:Use advanced de-escalation techniques to manage acute distress and reduce risk.
• Community-Based Support:Provide intensive home-based care to prevent A&E visits and inpatient stays, boost family/community integration, and build resilience and life skills in CYP.
• Shared Caseloads:Manage a small caseload within a multidisciplinary team, offering mutual support and ensuring comprehensive care delivery.

Operational Hours:
This role includes evening and weekend shifts (8 am–10 pm, 7 days a week).
