Summary


We are looking for an enthusiastic Occupational Therapist to join our dynamic Community Therapy Team. The successful candidate will provide assessment and treatment to a diverse range of patients who present with complex conditions. The candidate will be considered for progression to a Band 6 dependent on completion of competencies.
• To hold responsibility for your own case load, working without direct supervision.
• To actively participate in regular formal and informal supervision and training, clinical reasoning sessions, peer review, case conferences and reflective practice with a senior Occupational Therapist.
• To undertake all aspects of clinical duties as an autonomous practitioner.
• To be professionally and legally accountable for all aspects of own clinical practice and work.
• To undertake assessment of patients and use clinical reasoning skills to provide appropriate intervention and recommendations.
• To be responsible for providing occupational therapy assessment and treatment plans for patients with complex conditions.
• To assess and manage clinical risk within own patient case load.
• To ensure the delivery of a coordinated multidisciplinary service and integrate occupational therapy intervention into a patient’s multidisciplinary treatment program. This will include discussion of individualised patient care and treatment packages, patient progress, and involvement in discharge planning.
• To be responsible for the safe and competent use of all equipment used within the role including manual handling equipment, ADL equipment, and other patient appliances and aids. Check all equipment prior to use and report any faults to the relevant authority.
• To be responsible for organising and planning your own caseload to meet service and patient priorities

• To perform occupational therapy assessments of children, young people (CYP) and patients who have diverse physical and cognitive presentations.
• To formulate and deliver an individual Occupational Therapy treatment program based on a sound knowledge of evidence-based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills including manual techniques, exercise prescription, patient education, and other alternative options.
• To formulate soundly reasoned prognoses and recommend best course of intervention, developing negotiated patient centered goals.
• To assess patient, child/YP and families understanding of treatment proposals, gain valid informed consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
• Evaluate patient progress and treatment programs, reassess, and alter treatment programs as required, using recognized outcome measures, assessment tools and clinical skills.
• When working in the community, working as a lone practitioner with telephone support from a senior therapist if required.
