Crynodeb
We are looking for an experienced practice nurse to provide a high standard of practice nursing care for patients as well as providing nursing assistance to the doctors and other members of the primary healthcare team. You will be supported by a Senior nurse to deliver care within the boundaries of the role, focusing upon supporting patients to be healthy, monitoring long term conditions, health prevention and screening activities. You will be working collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.
