Slough OPMH has an exciting opportunity for a Community Psychiatric Nurse who wishes to join our dynamic Home Treatment Team. You will work alongside a very experienced clinical team and take responsibility for the assessment and implementation of intensive care and treatment in the home of people with mental health needs, as well as people with dementia.

You must have specialist knowledge of the care of older adults with mental health problems and mental health crises. You will have a good understanding of Mental Health Framework and legislation in addition to clinical risk policies and procedures is desirable.

The service operates 7 days per week, so flexibility with regard to unsocial hours/ shift patterns will be required. You will be required to travel within the community for the performance of your duties.

We are a friendly, dynamic and supportive team. You will be actively encouraged to develop professionally in your role within the team, and have regular access to supervision and peer support in your role.
• Participating as a member of a multi-disciplinary team managing a caseload of patients
• To undertake assessments of patients, involving their carers and family members in the patient's care and care plans.
• To be responsible for the formulation, execution, and evaluation of care plans for a designated number of patients who may present with significant complexity, in consultation with the patient and the multi-disciplinary team.
• The post holder will direct and supervise Support Workers and/or students.
• Post holder will provide advice to other members of the service and agencies on the needs of service users.

FOR SECONDMENT ROLES ONLY - If the secondment role becomes permanent, the successful candidate may be offered the permanent position.

The "must haves" for you to be considered for this role:
• Have the relevant professional qualification for a Mental Health Nurse
• Able to work flexibly on a shift pattern to cover CRHTT Service as required and the ability to travel independently between locations to fulfil the requirements of the position.
• Previous knowledge and experience of working in a care environment and with service users who have complex mental health needs
• Good understanding of the relevant legislation including the Mental Capacity Act / Mental Health Act
• The ability to assess risk in both routine and urgent situations

For this special opportunity, you will require:
- Good interpersonal, communication, observation, and reporting skills
- Able to empathise, be supportive and sensitive to the needs of others as well as being able to cope with distressing circumstances

HOURS:37.5 hours (unsocial hours may be required)
