Full Job Description: Maintain clear, compassionate, and professional communication with patients to ensure understanding of medication regimens, address queries, and promote adherence Liaise effectively with GPs, nurses, pharmacists, and administrative staff to support safe prescribing, accurate medication administration, and continuity of care Promptly escalate concerns or discrepancies related to medicines to the appropriate clinical staff to uphold patient safety and best practice Manage and streamline the repeat prescription process and co-ordinate requests for acute items Arrange relevant monitoring to support clinical reviews Run searches to identify patients due for medication reviews and arrange any relevant monitoring such as blood tests, blood pressure checks etc Run searches for patients on high-risk drugs requiring more frequent monitoring and identify any issues i.e. non-compliance and escalate to the clinical team Monitoring patient test diary on our clinical system EMIS to facilitate continuity of care Perform data quality checks to ensure timely and appropriate reviews Liaise with clinical staff, patients, and community pharmacies Support quality improvement initiatives, audits and medicines optimisation targets Provide administrative support to the management team e.g. submitting claims for prescriptions and drug monitoring Support ongoing rollout of the Electronic Prescription Services (EPS) Support patients with signing up to the NHS Wales App to improve usage Assist with creating relevant content for social media/the practice website Work within scope of role as outlined within medicines management standard operating procedures Aim towards medicines reconciliation, actioning requests from Secondary Care as the role is developed
