Your Role

You will provide tailored, empowering support to young people aged 1625 who are currently or have previously been in the care of the Local Authority, helping them navigate their transition into adulthood with confidence and stability. As part of this role, you will also contribute to the delivery of group-based activities and workshops, and actively support the Care Leavers Forumensuring young people’s voices are heard, valued, and central to shaping the services they receive.

About You

You will bring the skills and sensitivity needed to build trusting, creative, and empowering relationships with young people aged 1625, supporting them through a safe and confident transition into adulthood. You will have experience working alongside young people with complex needs, offering consistent, compassionate support including accompanying them to appointments to help them access the care they need.

Your experience working with care-experienced young people will give you insight into the challenges they may face, and you will be committed to amplifying their voices both individually and in group settings ensuring their lived experiences shape the services and support they receive.

About Us

You will work collaboratively as part of a multidisciplinary team including Personal Advisers (PAs), Intensive Support Workers (ISWs), Progression Coaches, and Education, Employment and Training (EET) Workers to provide wraparound, relationship-based support to some of our most vulnerable young people. These young people are often transitioning from residential or semi-independent living and may have experienced significant instability.

This role is part of an exciting and evolving service model. Together, the team will ensure young people receive holistic, timely, and trauma-informed care that supports their emotional wellbeing, independence, and long-term success.
