To work as a member of the Adult Integrated Care Dietetic Team in community settings across Lambeth and Southwark To provide dietetic care to adult patients who are undernourished requiring support with their oral nutritional intake. This includes patients who are malnourished or at risk of malnutrition, those with pressure ulcers and patients requiring nutritional support due to illness, long term conditions or social factor To communicate and work closely with members of the health and social care teams, general practice teams and the voluntary sector to ensure malnourished patients receive the appropriate care and support in the community To visit patients whom are housebound at home, and visit Care Homes or Intermediate Care Centres in Lambeth and Southwark as required. To collaborate with other teams providing care and support to patients in these settings including integrated care teams and care home nursing teams To provide an outpatient community clinic for patients whom are malnourished, prescribed oral nutritional supplements or seek dietetic advice relating to weight reduction, diabetes or other diet related concerns. To communicate effectively with the GP clinical team and improve referral pathways to the clinic To work closely with acute colleagues across Lambeth and Southwark including Guys and St Thomas acute dietetic teams To contribute to improving pathways of patient care including collaborative work with acute and community multi-disciplinary teams to support improved pathways of patient care from hospital to home e.g. frailty and long term conditions To contribute to the development of educational resources for patients and healthcare professionals, and deliver training to GPs, community multidisciplinary teams, care home staff and carers on malnutrition screening and nutrition support To provide expert dietetic advice to the multidisciplinary team through attendance and participation in multidisciplinary team meetings at community healthcare sites To follow local guidelines and evidence based practice when providing dietetic assessments and care plans, including the appropriate prescription of oral nutritional supplements To participate in regular research and audit in line with departmental and MDT procedures To develop positive partnerships and relationships with local health, social care and voluntary teams across acute and community settings To work in line with team standard operating procedures and policies including record keeping and outcome reporting To provide clinical supervision to band 5 dietitians and to support the development and provide mentoring for junior staff in the team including band 5 dietitians, advanced dietetic assistants and student dietitians To be the lead for student training and practice education in the Adult Integrated Care Dietetic Team
