Part-Time (15 hours) Band 6 Occupational Therapist – Frailty Service
We have a vacancy for a part-time Band 6 Occupational Therapist within our Frailty Service, based at Abel Smith House, Stevenage.

Although this post is part time there may be an opportunity to increase hours up to full-time, with a potential split post across other therapy pathways such as Discharge Home to Assess or Core Therapy.
About the Frailty Service:
Our service aims to improve outcomes for patients living at home with moderate frailty through comprehensive Geriatric Assessment. Working closely with Specialist Frailty GPs and therapists, our multi-disciplinary team focuses on assessing medical, psychological, social, environmental, and functional needs to:
· Reduce hospital admissions for frail patients
· Proactively identify falls and frailty
· Enable personalised treatment and long-term care planning
· Provide early access to advice, guidance, and local support resources
Role Details:
· Join a supportive therapy team covering Frailty, Discharge Home to Assess, and Core Therapy pathways
· Based at Abel Smith House, Stevenage with travel required to patients’ homes across East & North Hertfordshire
· Opportunity to work collaboratively with a skilled, multi-disciplinary team
Main duties of the job
The post holder will deliver assessment, intervention and advice to patients with various needs and health conditions in their own homes
The post holder will provide expert advice, guidance and information to health and social care professionals, carers, and relatives.
The post holder will participate in multi-disciplinary team meetings including partners in mental health and the voluntary sector.

You will support your team to ensure that patients receive high quality occupational therapy
You must have a relevant Occupational Therapy qualification and HCPC registration
You will have at least 2 years qualified experience.
You will work alongside the rest of the MDT to ensure patients achieve their goals and receive therapy input in a timely way
You must hold a full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).
