Crynodeb
The Specialist Palliative Care Team (East) is based in the Wrexham Maelor Hospital. The team provides specialist support, care and advice to patients and their families living with life limiting illness, in a variety of settings, including the patients own home, acute and community hospitals and residential and nursing homes. The team also works in close partnership with the local adult hospices in North Wales and across the border.
