Crynodeb
To offer support in the implementation of Positive Behaviour support plans and to provide intensive support to patients, carers and care providers to prevent/ reduce admission to inpatient units. Offer early/timely intervention when there is deterioration in a patients condition by working with staff and the patient within their home offering assessment and intervention, including practical hands-on support. To be part of the team providing a response to potential placementbreakdown.
