In this role, you will provide assertive outreach support to reduce rough sleeping, conducting thorough, needs-led assessments and developing person-centred support plans. You will create and implement risk assessments and management plans, work flexibly to respond to the needs of clients and services, and facilitate supported reconnections to appropriate services and areas. Your work will involve assisting clients in obtaining welfare, housing, and related rights, advocating for them to ensure all housing opportunities are explored. You will refer clients to specialist services such as substance misuse, mental health, welfare rights, medical, and legal services, and support those in temporary accommodation to manage their tenancies and prevent a return to rough sleeping. Additionally, you will build relationships and develop referral arrangements with partner organisations, liaise with statutory, voluntary, and church-based organisations, and collaborate with various agencies involved with clients. Engaging with the local community, involving clients in service design and delivery, and maintaining accurate client data and reports will also be key aspects of your role. Compliance with Turning Tides’ policies and regulatory requirements, promoting equality and diversity, and participating in personal development and organisational activities are essential to the position.
