Crynodeb
Provide behaviour and lifestyle change support to patients across a wide range of physical and low level mental health conditions in both individual and group settings. Promote self-management and independence by providing personalised support that enables individuals to take control of their health, make informed decisions, and improve health and wellbeing outcomes. Signpost and refer patients to relevant services for further support where appropriate. Complete a Personalised Care Institue (PCI) accredited coaching skills training course, where necessary, and engage in ongoing professional development including regular coaching supervision. Stay up to date with local and national health issues, services and resources. Collaborate with other teams and staff members to identify and enhance the scope of the role in line with evolving patient, practice and PCN needs. Actively promote the Health and Wellbeing Coaching service among patients and staff, seeking opportunities to support practices and patient care within the scope of the role. Contribute to the evaluation of the impact of Health and Wellbeing Coaching.
