The role will establish and oversee a team of Community Network Connectors who offer short-term, practical, and emotional support to individuals facing temporary life challenges, such as bereavement, illness, or a recent loss of mobility. This support is intended to build confidence, reduce social isolation, and promote self-care and community connection.
The Community Network Lead will ensure the model is firmly rooted in the local community, while also aligning with the strategic objectives of the local health and care system, including reducing demand for urgent care, supporting earlier interventions, and delivering care closer to home. By working closely with statutory and voluntary sector partners, the postholder will play a key role in building trust, coordinating referral pathways, and integrating the Community Network into wider neighbourhood delivery models.
The role combines strategic leadership with hands-on delivery. It requires a clear understanding of person-centred practice, community development, and health inequalities, as well as experience in operational management, service design, and staff and volunteer development. The postholder will lead on recruitment and training of Community Network Connectors, establish robust governance and safeguarding frameworks, and ensure that delivery is safe, responsive and impactful.
As the programme develops, the Community Network Lead will monitor performance, capture learning, and ensure continuous improvement. They will be responsible for evidencing impact at individual, community and system level, and will contribute to strategic planning and funding proposals to support the long-term sustainability and growth of the model.
Ultimately, this role will help create a sustainable, community-rooted support offer that empowers individuals, strengthens local connections, and contributes to a more integrated, compassionate and preventative health and care system.
