Crynodeb
The post holder will be an integral part of the Urology Team. Working under the guidance of a registered health professional, this role supports people living with cancer who have been assessed as having noncomplex needs and who are able to self-manage their care needs. The aim of the role is to make sure that people living with cancer can access the right support, at the right time. The support worker will act as a keyworker, identifying a persons holistic needs, coordinating a persons care, navigating the health and social care system on their behalf and using a dedicated database (My Medical Record) to track patients through their follow up and surveillance ensuring that milestones are met.
