Rough Sleeping Navigator needed in South East London Paying £22.78 per hr ref OR9744

Full time hours on a temporary basis

  • We are seeking a compassionate and proactive Rough Sleeper Navigator to support individuals experiencing street homelessness. The Navigator will work intensively with rough sleepers to build trust, identify needs, and coordinate access to services that support recovery and sustainable accommodation.

    Key Responsibilities
    • Outreach & Engagement: Conduct regular outreach to identify and engage with rough sleepers, building trusting relationships to encourage participation in support services.
    • Case Management: Develop and maintain personalised support plans, working closely with individuals to address barriers to housing, health, and wellbeing.
    • Multi-Agency Coordination: Liaise with housing providers, health services, substance misuse teams, mental health professionals, and other relevant agencies to ensure a joined-up approach.
    • Advocacy: Act as an advocate for clients, supporting them to navigate complex systems and access appropriate services.
    • Monitoring & Reporting: Maintain accurate records of engagement, outcomes, and progress. Provide regular updates and reports to funders and stakeholders.
    • Crisis Intervention: Respond to urgent needs and situations, ensuring safeguarding protocols are followed.
    • Service Development: Contribute to the development of local strategies to reduce rough sleeping and improve service delivery.
