This is an interesting, exciting and challenging opportunity for an enthusiastic person to work within a ‘Locality’ working model. Mainly based in the West locality team, but will have opportunity to work across West Lothian where required. The role also includes working with the integrated first point of contact team, within the central ‘Integrated Access Point’ (IAP).
This is a recently designed service initiative with joint working across Health and Social Care partnerships. The team deliver a service to a range of individuals who require support to maintain their safety and independence within the community. The role includes responding to the demands of a busy service by having the ability to prioritise, be organized, follow procedures, have excellent communication and good report writing skills. The successful candidate must also demonstrate the ability to work as part of a health and social care team.
A recognised qualification in social care is an advantage and to be able to undertake essential training relevant to the post and in accordance with service standards.
As part of the role the successful candidate will be required to have a good understanding of Self-Directed Support and the importance of completing outcome focused assessments. Training will be given as part of the role.
The role includes having a current driving driving-licence and have the ability to travel throughout West Lothian.
This post is considered Regulated Work with children and/or protected adults under the Protection of Vulnerable Groups (PVG) Scotland Act 2007. From 28 February 2011 preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council
