TheICNMedicinesOptimisationService(ICNMOS)isagroupofsixspecialistclinicalpharmacists and one Medicines Management Technician (MMT)workingacrosssixclinicalnetworksinCroydon.TheICNpharmacistssupportvulnerableandhouseboundpatientswithlongtermconditions,tousetheirmedicinessafelyandappropriatelyandtomanagetheirmedications independently for as long as possible.
Theserviceaimstoreducemedicines-relatedhospitaladmissionsandmedicationincidentsanderrors.By promoting better use of medicines in an at-risk population, the service also supports a reduction in inappropriate polypharmacy andmedicines waste.
ICN/ Neighbourhood core team members include GPs, pharmacists, community nurses, social workers and personal independencecoordinators.ICNcoreteamsattendweeklymulti-disciplinary“huddles”ateachGPpractice toidentifyhighrisk patients anddevelopcareplans.

TheseniorLocalityPharmacistfortheICNwill:
§ Provideexpertleadershipandspecialistprofessionaladviceonmedicinesmanagementwithinadesignated locality ICN and virtual wards.
§ Beresponsiblefordevelopingandmaintainingaservicetosupportmedicinesoptimisationissuesforpatientsindomiciliarysettings.Undertakemedicinereviewsandsupportpatientswiththeir medicinesindomiciliary,carehomeandfallsclinicsettings.
§ Makerecommendationsandsupportthedevelopmentofnecessaryframeworksforensuringsafemanagementofmedicinesthroughrelatedstrategy,policyandtrainingwithintheTrustand integrated community networks.
§ Superviseband7andband6juniorpharmacists,techniciansandpre-registrationgraduatesworkingin community services and ICNs, as required.
§ AttendandcontributeeffectivelytoICNandothermultidisciplinarymeetingse.g.GPhuddles,includingidentifyingindividualssuitablefordiscussion.
§ HaveaknowledgeofotherserviceslinkedtotheICNandtheirpartnerorganisationstoenableappropriate referral and signposting.
§ Beresponsiblefordevelopingsystemstocollatedata,casestudiesandotherrelevantinformationtoenable regular reports to be generated.
