Crynodeb
To undertake all aspects of practice as an autonomous practitioner including professional and legal accountability for own work. To carry out holistic needs-led assessments and manage a complex caseload, with responsibility for team or onward referral as required, acting as the care manager for identified individual service users. To liaise with various multi professionals, clients and carers, in situations which can involve highly complex, sensitive and occasionally contentious information using well developed communication skills, i.e. case conferences To liaise with the Associate Director of Nursing & Professional Practice for professional supervision and consultation in accordance with best practice. To be responsible for maintaining own competency to practise through CPD activities, maintain a portfolio which reflects personal development. To be responsible for the identification and facilitation of the specific abilities and interests of team members, through performance review and personal development plans To be responsible for the maintenance of professional status in line with N.M.C requirement. To be a registered Practice Supervisor and Assessor within the Care Trust responsible for teaching and mentorship of professional students to graduate, and postgraduate level, on profession-specific skills and knowledge within core clinical areas. As Community Nurse Clinical lead you would also undertake the role of sign off Practice Assessor as required by the NMC. To utilise prescribing rights to improve the effective access to medicines for patients on the case load in a timely and appropriate manner that will enhance the care delivered.
