We have an excellent opportunity for a social worker to join the Older People and Physical Disability Team in Calderdale which sits in the portfolio for long term personalised support. While the posts sit within the Central Team there is flexibility in home working and access to touch down hubs to support the future development of community networking.
Calderdale Adult Services have a dedicated older people and physical disability service based across five locality teams. You will join a supportive, compassionate and knowledgeable team making a real difference in people’s lives. Working alongside individuals from 17 years up, families, informal carers and external agencies. You will hold a varied case load which may include supporting young people transitioning to adult services, completing Care Act Assessments, support planning and reviews within the community and residential settings. Coordinating hospital discharges, Continuing Healthcare assessments, carers assessments, supporting individuals around homelessness, substance / alcohol misuse. Building trust with individuals who may be reluctant to accept support. Work with direct payments and support with financial assessments where required. Safeguarding enquiries and Court of Protection work.

The council is passionate about engaging with adults, carers, and their families to ensure a personalised approach is at the heart of every conversation. As a Social Worker you will have a pivotal role as part of the team supporting individuals to maximise their potential and improve their quality of life and overall wellbeing.
As a social worker, you could also provide peer support, be a mentor, work base supervisor or practice educator for social work students, apprentices, newly qualified social workers and service coordinators.
The role requires you to be:
• An effective communicator and active listener as you will support individuals, carers, and families which requires understanding their lived experience.
• Excellent organisation skills as you be required to manage your own time and casework to ensure tasks are prioritised daily in a busy locality team.
• Confident in working with other stakeholders as part of a multi-disciplinary team.
• Good record keeping as you will be required to make accurate case recordings, type factual assessments and detailed support plans.
• Ability to assimilate information and make professional judgements.
• A caring and compassionate person as you need to have a willingness to help others and demonstrate emotional empathy to be successful in this role.

• A background in the Adult Social Care Sector who are looking for an opportunity to develop skills in assessment and increase knowledge around legal frameworks.
