The posts will be in the Department of Obstetrics & Gynaecology at the North Middlesex Hospital and the appointee will join the rota for ST3-7.

The new appointees will join a busy and supportive department with a strong commitment to high standards of patient care. There will scope in the job plan to undertake ATSM in advanced labour ward/ antenatal practice, Fetal Medicine or high risk pregnancy if you are eligible and support will be provided for completion of core training log book and application for CESR.

Gynaecology

The North Middlesex Hospital is in a state of rapid change with a brand new “purpose-built” Accident and Emergency Department which houses a Gynaecology Emergency Room. We have an active Early Pregnancy Assessment Unit/ Emergency Gynaecology Unit, with two trained nurse ultrasonographers plus 3 Consultant sessions, two ultrasound machines with pulsed colour Doppler facilities, and daily beta-hCG assays.



The hospital has been designated as a Cancer Unit within the “North Central” London Oncology network.



There are modern, up-to-date facilities for minimal access gynaecological surgery including video cameras, laparoscopes, hystero-resectoscopes, Novasure endometrial ablation. The success of the service has led to increased demand for minimal access surgery from GPs and patients. There is two weekly out-patient hysteroscopy clinic.



There is at present one weekly gynaecological urodynamic session. Enfield and Haringey Health Authority employ a senior nurse incontinence advisor and there is an active incontinence support group with a senior physiotherapist.



Obstetrics

General description

Currently the department caters for around 5000 deliveries per year. Large proportion of our women are from ethnic minorities with associated problems such as pre-eclampsia, sickle cell disease, thalassaemia, HIV and substance abuse, although the perinatal mortality rate is currently around 6 per 1000.

The new unit has opened in November 2013 and has a brand new Midwifery-led unit and a new Labour ward with 18 delivery suites, 3 high dependency rooms and 2 theatres

Gynaecology

For emergency Gynaecology the post-holder should be competent in performing

· Surgical Management of miscarriage

· Diagnostic laparoscopy
• Laparotomy for ruptured ectopic pregnancy



Training will be given (if necessary) in Laparoscopic Salpingostomy/ectomy

The successful candidate may attend both general and specialised gynaecology clinics. Any complex cases or patients who will need surgery should be discussed with the Consultant.



Obstetrics

The successful candidate should be able to give basic antenatal and postnatal care and advise patients. Complex problems should be discussed with the Consultant.

On Labour ward the Registrar should be proficient in:
· Management of Normal Labour• Instrumental Deliveries
• Suturing of episiotomies and tears
• Manual removal of placenta
• CTG interpretation and Fetal Blood Sampling
· Caesarean Sections
All Labour Ward sessions are covered by a Consultant who will lead the Ward Round and who will supervise the post-holder.
TEACHING
It is expected that the appointees will take a full part in delivering the undergraduate curriculum, both to the junior ST and medical students attached to the department.



Miss Schahrazed Rouabhi is the RCOG Tutor.

The post holder will be expected to actively contribute to the postgraduate teaching programme which occurs every Friday afternoon as protected teaching time for trainees. There are weekly CTG training sessions and regular labour ward ‘rehearsals’ of obstetric emergencies.



There are also regular General Practitioner teaching sessions.


MEDICAL ADMINISTRATION
The appointee will be expected to take part in the administration of the department and work with colleagues, including the Service Director, on matters relating to the clinical service. The appointee will also be encouraged to take part in management concerning the hospital as a whole.



CLINICAL AUDIT

The appointee will be required as part of their duties to take part in the audit and clinical effectiveness activities in the Trust and to attend the Audit and Effectiveness half days and to ensure that trainees within the department also take part in these activities.
