Post: Practice Nurse Accountable to: Practice Manager and GP Partners Place of Work: Elm Lane and Chapeltown Surgery Job Purpose To deliver a high standard of nursing services to the practice population across both sites; including treatment, screening and advice in accordance with the N.M.C code of professional conduct. Working with the GPs and nursing team to meet medical needs as appropriate. Key Responsibilities: Assessing the nursing needs of patients, providing appropriate care and treatment in conjunction with the GPs, or independently, as required and according to practice policy and protocols. Have a range of clinical skills developed through experience and deal with patients needs with competence and according to practice policies and protocols. These duties will include: Wound care-assessment and management Ear Syringing Venepuncture Pathology samples-send to laboratory and/or test in surgery as appropriate Hypertension management Adult and childhood immunisations Travel health advice and immunisations Cervical cytology New patient registration medicals Contraception Advice and annual checks Advice on general health care on minor ailments with referral to the GP as necessary Organise and make home visits to the elderly or other patients for routine health checks or treatments. Chronic Disease Management Assist in the provision of nursing care to patients with chronic diseases, as per practice policies and protocols, including: Asthma and COPD-using Spirometry Diabetes Coronary heart disease Hypertension Rheumatoid Arthritis Learning Disabilities Monitoring and maintaining patient registers for the above. Performing audits on the above. Assist in maintaining the practice protocols on all of the above. Record Keeping Document the assessment of need, delivery of care, the evaluation of the outcome of care and make changes to the care plan for the patient, ensuring that:- An appropriate entry is made in the patient medical records both paper and computer records. Appropriate forms are completed as necessary. General Treatment Room Duties: Ensure that Infection Control procedures are adhered to. Ensure that instruments are cleaned, sterilised and stored in accordance with infection control policy. Ensure the Treatment rooms are kept clean and tidy. Manage, order and restock medical supplies in the treatment rooms and consulting rooms. Manage vaccine ordering, including cold chain storage and distribution, and fridge monitoring. Ensure all medical equipment is properly cared for, regularly services and any faults reported as soon as possible. Information Technology Input of clinical data obtained during a consultation, including immunisations, BP etc. Retrieval of relevant medical information from the patient database for the purpose of medical audit or practice research. Education and Training Maintain NMC registration, following the professional code of conduct. Maintain the PREP standards as outlined by the NMC, ensuring that the nurse is aware of up-to-date research based evidence to support her nursing care and to ensure clinical effectiveness. Prepare for and take part in an annual appraisal. Participate in the education and training of students and the induction of new members of staff. Attend practice meetings, and appropriate training courses as agreed by the GPs. General Practice Policies and Procedures: Adhere to all practice policies and procedures. Contribute to practice targets both local and national within the disease management agenda e.g. QOF, prescribing incentive schemes, National benchmarking Deal courteously and efficiently with patients, maintaining strict confidentiality of patients and partners information. Cooperate with other members of staff, not jeopardising the work, aims or reputation of the Practice. Undertake any other duties which may be assigned by the partners or Practice Manager as can reasonably be expected.
