Location:Central Basildon PCNSalary: Competitive, based on experience Hours:Part-time -preferably Monday, Wednesday and Thursday. We are seeking a highly skilled and compassionate Advanced Nurse Practitioner (ANP) to join our Primary Care Network (PCN) and provide dedicated support for care home residents. This role is essential in delivering proactive, patient-centered care, ensuring residents receive timely assessments, treatment, and ongoing management of their health conditions. Key Responsibilities Triage and clinical assessment of patients presenting with acute symptoms (e.g., respiratory infections, urinary tract infections, rashes, minor injuries, etc.) Work collaboratively with GPs, pharmacists, and multidisciplinary teams. Support medication reviews, ensuring safe prescribing practices. Lead on preventative care, reducing unnecessary hospital admissions. Offer guidance and training to care home staff on best practices. Maintain accurate clinical documentation and contribute to quality improvement initiatives. Requirements Registered Nurse (NMC) qualification with Advanced Practice training. Independent Prescriber (V300) qualification (essential). Experience in elderly care, long-term conditions, and complex case management. Strong clinical decision-making and leadership skills.
