This exciting new post will be working in a newly established Frailty service at the North Middlesex University Hospital NHS Trust. This role is part of a key development in delivering specialised hyperacute Frailty input to older patients living with Frailty presenting to ED. The post holder will be included in the 7-day working. The successful candidate will deliver direct patient care to patients across Urgent and Emergency Care as part of the GEMS (Geriatric Emergency Medicine Service) team and contribute in the delivery of timely, specialist input to patients with Frailty.

Thisis a full time Band 3 post working in a newly established Frailty service at the North Middlesex University Hospital NHS Trust. This role is part of a key development in delivering specialised frailty input to frail, older patients presenting to ED. The successful candidate will work across Urgent and Emergency Care within the GEMS team and contribute in the delivery of timely, specialist input to frail patients; effecting appropriate admission avoidance and early, pro-active CGA for patients who may need admission to appropriate wards.

The post holder may be included in the 7-day working as required.

The successful candidate will be organised, proactive, and compassionate, with excellent communication skills and the ability to work flexibly in a busy and evolving clinical environment.

· Maintain and improve the patient’s mobility using appropriate equipment and handling techniques

· Assist the patient to be as comfortable as possible, i.e., help with moving and positioning the patient using appropriate equipment

· Assist with checking pain levels and requesting pain relieving medication on patient’s behalf

· Involve the patient in shared decision-making and obtain their consent before undertaking nursing procedures

· Prepare appropriately for the activity to be undertaken

· Support and encourage the patient to meet their own health and wellbeing needs

· Undertake care activities to meet individuals’ health and wellbeing needs, once competence demonstrated:

· Work co-operatively as part of the multi-disciplinary team, helping nursing, and other clinical staff in routine work associated with patient care

· Support qualified nurses in the implementation of programmes of care within the limits of the Healthcare Assistant’s knowledge and competence

· Provide comfort, reassurance, and support to patients and/or their carers if anxious or distressed.

Education & professional Qualifications:

Literacy and numeracy skills

NVQLevel 3 or equivalent care certificate level of experience



Experience:

Experience of working with adults within a health care environment NHS or private sector



Has understanding of the different values, cultures and beliefs patients may present with and be able to respect this adapting care delivery according



Clinical competency – wound care/invasive devices i.e., catheter, PEG care

Use assistant aids and equipment for care provision.

Demonstrable ability to meet Trust values.
