About the role:
Home Support is an early help and prevention service for citizens (age 50+) that enables and provides the support and practical assistance an individual may need in their day-to-day life to live at home with confidence, in good health, independently and safely.
Home Support services are currently delivered in four of the thirteen Powys localities.
Since the start of the project in 2018, ongoing data collection, collation, and evaluation has served to secure continued project funding and provided extensive local evidence demonstrating the impact and difference that Home Support services have made to individuals’ lives.
Currently, two of the four services are permanently funded and two are funded through the Powys Regional Partnership Board (RPB).
The post holder is accountable to the Strategic Commissioning Manager, Live Well projects and will work closely with the Live Well and Age Well Team.
The postholder will provide high quality, effective and efficient commissioning support and project management to the Strategic Commissioning Manager, Live Well projects to deliver on key workstreams, projects and partnership work activities as part of the whole commissioning cycle.

To support Strategic Commissioning Managers in achieving the expected and desired outcomes in given areas through effective commissioning activities.
The post holder will have a key role in the contract management, marketing and communications, and partnership working of/within current RPB project funded Home Support services.

Information management skills and abilities and use of IT systems that support the review and analysis of service and project level quantitative and qualitative data.
Ability to prioritise and manage a busy and complex workload.
Self-motivated, adaptable and flexible with an ability to motivate others through periods of significant change.
Be determined, persuasive and enthusiastic.
Be proactive, and to be able to act on their own initiative.
What you will do:
Work closely with the Strategic Commissioning Manager, Live Well projects.
Provide high quality, effective and efficient project management to the Strategic Commissioning Manager to deliver on key projects and partnership work activities.
Support, monitor and develop the existing Home Support services.
Lead and provide high quality, effective and efficient project management to the Strategic Commissioning Manager to deliver on key projects and partnership work activities.
Lead and support development of the project, including communication and liaison with stakeholders and partners.
Responsible for maintaining, developing and building partnerships internally and externally to influence and support the achievement of key initiatives.
Gather, collate and analyse both quantitative and qualitative data to measure service impact and inform service development.
Draft reports on the project.
Convene, facilitate and/or administer project and working groups.
This position has a requirement for a Standard DBS Check
