Crynodeb
To provide and develop a comprehensive, patient-focused, clinical pharmacy service to different specialties promoting the safe, appropriate and clinically effective use of drug therapy. To frequently communicate complex drug or medicine related information to patients andcarers, tailoring the information to the patients specific needs. The patients may have language difficulties, physical or mental difficulties. Provide patient focused pharmaceutical care to patients, to achieve definite outcomes that improve patients quality of life. To maximise patient care on defined areas within the Trust by planning, managing, monitoring, advising and reviewing specialist pharmaceutical care programmes for patients. Evaluate the clinical appropriateness, safety and legality of prescriptions based upon the patients diseases, co-morbidities, other medication and treatments and liaise with the prescriber to make agreed changes as appropriate.
