To be an integral part of East Berkshires Continuing Healthcare and Funded Nursing Care Team. The postholder will implement the National Framework for Continuing Healthcare and NHS Funded Nursing Care. To complete high quality holistic assessments, reviews and re-assessments in a variety of settings i.e., nursing home and community to enable application of the NHS National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care. Face to face reviews/assessments will be required. Work as part of a multi-disciplinary team, acting as a resource to provide nursing advice in relation to appropriate placement and management of clients. Provide accurate advice and information to all regarding recommendations following completion of assessment process. To manage a generic caseload of clients following their placements for Continuing Healthcare and function as a Duty Nurse within the Continuing Healthcare Team. These duties will require the postholder to work with other organisations. Support teams to ensure that the portfolio of tasks/projects is planned, managed and delivered effectively. Support and inform the targeting of resources, monitoring, implementation and evaluation of the tasks/projects by providing high quality support including complex information and analysis, communications and stakeholder management. Ensure accurate and open communication and co-ordination with a range of organisations and individuals, researching and drafting correspondence and papers and ensuring the management of specific tasks, lead reporting and analysis across a range of specialities, functions and projects. Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally.
