Deliver group sessions on pregnancy and infant care
Help women engage with maternity services early
Work collaboratively with midwives, GPs, and Family Hubs
Build trust with women from diverse communities
What your week will look like:

Support & Engagement: Conduct home visits, accompany women to antenatal and diabetes-related appointments, provide translation assistance, and offer reminders via text or calls.
Health Promotion Activities: Signpost to and support group sessions on healthy pregnancy topics such as physical activity, nutrition, smoking cessation, and managing gestational diabetes.
Community Outreach: Build relationships with local communities, attend drop-ins or outreach events (e.g. at refugee centres), and promote access to services like Healthy Start.
Multi-agency Coordination: Collaborate with GPs, midwives, and public health teams through meetings and referrals to ensure integrated, holistic care.
Monitoring & Administration: Track outcomes (e.g. appointment attendance, CO readings) and complete reports and case notes for ongoing evaluation.
What we’re looking for:

A Level 3 qualification (or equivalent experience) in health, early years, or public health
Experience working with vulnerable families or in public health
Warm, trauma-informed communication style
Strong organisation and safeguarding awareness
