An ability to effectively communicate routine information to patients/carers where co-operation is required and where there may be barriers to understanding An ability to communicate and liaise within a range of settings to other health professionals, agencies and voluntary organisations including: Community Nurse Team Leader Community Nurse Team GP and Practice Staff Multi-Disciplinary Teams Social Service Employees Patients families and carers Residential/nursing homes Voluntary and private care providers Responsibility and accountability Assisting trained nurses in delivering a high standard of holistic nursing care to patients in the home environment, within the dimensions of the post Communicate and liaise effectively with patients, carers and members of the primary health care team including statutory and voluntary organisations Implementing delegated, planned and appropriate aspects of nursing and social care unsupervised and being responsible for informing the Community Nurse Team Leader/Nurse in charge of observations relating to patient care.
