The SDFS team are looking for a senior discharge co-ordinator to support the flow of West Essex patients through the 2 older adult mental health inpatient wards at St Margaret's Hospital in Epping.
The successful candidate will work alongside the ward MDT, the SDFS team care co-ordinator and social care colleagues to ensure the discharge planning and processes are streamlined and completed in a timely way to prevent unnecessary discharge delays and prolonged admissions. The role will include attending ward rounds, length of stay meetings and CPA reviews as well as supporting the completion of MCA's, NHS Continuing Health Care (CHC) assessments, Self Directed Support (SDS) assessments and S117 care planning documents. The successful candidate can be from any professional background and will have a sound understanding of the Mental Health Act, local authority and NHS funding streams.
The successful candidate will be line managed by the SDFS community team and will have the opportunity to learn about the local community services available and how the wider service works to ensure a streamlined and safe transition for patients from the ward to the community based setting at the point of discharge.
The West Essex Specialist Dementia and Frailty Service (SDFS) is an assessment, diagnostic and support pathway for people with dementia of any age, as well as people over the age of 75 who have complex mental health needs and an associated age related frailty.
We work closely with our EPUT community specialist practitioners and Essex County Council colleagues, local organizations as well as the voluntary sector to support the holistic needs of the patient with a person centred collaborative approach.
The successful candidate will be trustworthy, positive about change and will be passionate about the services we provide and have excellent interpersonal skills and a zest for delivering the highest level of patient care and service delivery improvements.
The successful candidate will have excellent communication and documentation skills, be organised and able to prioritise workloads, be flexible and have a 'can do' attitude, be able to work independently as well as alongside patients and their families, the ward and community based staff as a valued member of both teams.
